- Provide direct patient care, including assessment, planning, implementation, and evaluation.
- Administer medications and treatments as prescribed by physicians.
- Monitor and document patient progress and responses to treatment.
- Collaborate with interdisciplinary teams to develop and implement patient care plans.
- Educate patients and families on health management and disease prevention.
- Ensure compliance with all healthcare regulations and standards.
- Complete certification documents and death visits.
- Create individualized care plans for geriatric patients in hospice.
- 15 days PTO, 7 holidays

- Salary: $95,000 - $105,000 per year, depending on experience
